What kind of senior services does Assisted Care Connections provide?
Assisted Care Connections is primarily a senior care placement service. We love helping seniors find independent living facilities, assisted livings, memory cares and certified family homes to fit their physical and budget needs. We can also help connect them with personal care service caregivers, skilled home heatlh, hospice, wound care, durable medical equipment, house calls providers and more.
Who can send seniors and families to Assisted Care Connections for help?
-Hospital discharge planners
-Doctors’ offices
-Rehabilitation centers
-Home Health/Hospices
-Personal Care Services Companies
-Friends, families
-Assisted Living, memory cares, independent livings, certified family homes
-Social Workers, Registered Nurses, Physical/Occupational Therapists, Aides, etc
Is there a charge for services?
No charge to seniors or their families for our services. Our network of senior care facilities and providers compensate placement advisors for their work. Laws do not allow providers to compensate for Medicaid referrals so Assisted Care Connections has provided a Medicaid availability list to assist Medicaid seniors and their caregivers in finding placement.
How can I send Assisted Care Connections a Senior or family that needs help?
-If a senior is currently on Medicaid or will need to apply for Medicaid- direct them to your local website that has a list of available Medicaid spots in your area.
-All other referrals can be instructed to call us for assistance
– With their permission, you can also call us and give the senior/family contact information for us to reach out to them
-You can utilize our secure fax to send a history & physical and medlist to 208-779-5423.
Can I get updates about the seniors I send for assistance?
Yes just be sure to specify how you want those updates if it be via email, phone or fax
What are the benefits to using Assisted Care Connections for Senior care needs?
The #1 benefit is having an experienced senior care nurse as a placement advisor. Only an experienced senior care nurse knows how a certain diagnosis, or a combo of complex diagnoses will impact care needs presently and in the future. Other factors such as family support or the lack of support, age, budget considerations, social/emotional needs of both seniors and families and covid restrictions – you want to be advised by someone who knows about ALL of it.
